THE REHABILITATION CENTER AT SCVMC

Santa Clara Valley Medical Center is located at 751 S. Bascom Avenue in San Jose, California. The hospital’s main campus is​ located near the 280 freeway at the intersection of Moorpark and Bascom Avenues. The visitor parking garage has entrances on both Moorpark Avenue and Renova Drive.

REFERRALS & ADMISSIONS

Santa Clara Valley Medical Center Rehabilitation treats patients with many different neurological diagnoses, and we admit to three specialized units. The third floor provides 22 beds devoted to spinal cord injury and general rehab, as well as 10 beds for the respiratory rehabilitation unit. The fourth floor provides 32 beds devoted to stroke, brain injury, and general rehabilitation. All referrals go through our admissions office, and include a thorough physician review, as well as case management review of diagnosis, medical condition, rehabilitation goals, funding requirements, and anticipated discharge disposition. We treat young patients, generally from 0 to 16, on our Pedi Rehab floor, which is managed by pediatricians, with rehabilitation consult.

All referrals are handled on a case by case basis, with a great deal of attention and customer service, which often includes an on site visit by one of our liaisons out in the field. Because SCVMC is part of a Level 1 trauma hospital, we have the resources to handle medically complex patients, including those on ventilators. Many of these medically complex patients begin their stay with us in the Respiratory Rehabilitation Unit; this intensive care setting allows a team of rehabilitation and surgical specialists to provide immediate admission and treatment.

As an academic medical center, we seek to provide information and resources regarding the care of neuro rehabilitation patients regionwide. We understand that interpreting admission criteria can be difficult, and the rules can change, so we welcome your phone call with a referral, or with general questions. This is what we do every day and we can help provide answers in this often complicated process.

  • Every patient highly evaluated to ensure they qualify for treatment
  • Rehabilitation physicians and clinical liaisons review the patient’s case in detail
  • Patient’s insurance carrier is contacted to confirm coverage
  • Patient officially accepted into rehabilitation
  • Discharge plan arranged and insurance details finalized

SUPPORTING FAMILY INVOLVEMENT

Our long experience with inpatient rehabilitation has clearly shown how important family and friends can be to the process. Since the time we treated our first patients more than 40 years ago, we have worked hard to provide a very welcoming setting for our patient's "support systems." We closely involve friends and family in support groups, recreational activities and outings, as well as in the rehabilitation process itself, including early goal-setting and frequent team meetings. Because our physician specialists are in-house, not just "on call," they are closely connected to the inpatient program and available and responsive to patients and families. This distinctive level of access on the part of exceptionally talented and dedicated physicians is one of the hallmarks of the rehabilitation experience at SCVMC.

ACCEPTED INSURANCE

Considered payer sources include: Medicare, Medi-Cal, Commercial, Health Maintenance Organizations (HMO), Workers Compensation, California Children’s Services (CCS), and private pay. Final authorization is at the discretion of the insurance company. Referrals are accepted from all sources, which may include hospital, physician/group, payer, or private pay. We work with all major payers.​​
