myHealth Online is an easy-to-use secure web site that gives you access to your health information. It is a customized version of MyChart. The myHealth Online website and MyChart mobile app allow you to view your test results, request prescription refills, make an appointment, and message your doctor directly from your computer or smartphone.

Download the myhealth mobile/MyChart App
You can get myHealth Online with the MyChart® application on iTunes or through the Android Market.
DOWNLOAD INSTRUCTIONS:
- Access the App Store or Android Market on your mobile device
- Search for 'MyChart' to locate the application
- Download the free MyChart application
- Launch the MyChart application and select Santa Clara Valley Medical Center as your healthcare provider
- Log in using your myHealth Online username and password
